Description:
The Maine Center for Disease Control adopted KIT’s prevention data system to streamline reporting and provide an improved reporting tool for process evaluation and monitoring. The Prevention Service was adopted with significant costs savings to the state. It acts as a single data-driven decision support service to strengthen collaboration between agencies. It is used in conjunction with the Healthy Maine Works Program and Healthy Maine Partnerships as an evaluation tool managed by the Office of Substance Abuse.

Sub-recipient Checklist Part 1 and 2

• Special worksheet modules/reports specific to Maine.
• State utilizes the reports for their SIG funding report requirements.

Quarterly Progress Report

• Special worksheet module/report that is required of Maine providers that receive SIG funding.

• Work plan reports used to track the status of selected state specific goals, health promotion categories, and objectives per work plan year.
• Maine has contracted third party evaluators to perform their technical support.
• All end users speak to the third party vendor for any support needs.
